The People Eater wears a 3-piece suit. There are holes on his vest for his nipples to show through. Between his nipples hangs a chain, with handcuff piercings on each nipple, and an odometer hanging as a pendant on the chain (apparently to use as a counter).
His shirt cuffs have cuff links that look like nipples. He is wearing a Soviet-style children’s gas mask over his crotch like a codpiece.
He wears a filigree nose cover. The Art of Fury Road book explains that he uses it to cover his leprous nose.
His right leg looks like it has elephantitis.
